What you should see on a valid letter

• Clinician full name and credentials (e.g. APRN)
• Florida license number (e.g. APRN########)
• Optional NPI
• Patient name matching the tenant
• Named animal(s) / type
• Issue date and validity (typically 1 year)
• Statement of disability-related need for the animal (not “just a pet”)
• Path to verify the clinician

• Whether the writer is a licensed professional and in which state
• Confirmation that an evaluation / professional relationship exists
• That the letter was issued for the named individual and animal(s)
